Panayiotopoulos syndrome and continuous spike-wave during slow sleep.

چکیده
A 6-year-old girl suddenly developed a loss of conciousness episode with head deviation to the left, generalized hypertonia, and clonic movements when waking up. Then, she presented vomit and bladder sphincter incontinence. Therefore, the Panayiotopoulos syndrome was suspected. Brain magnetic resonance imaging (MRI) was normal. The electroencephalography (EEG) performed after 48 hours of the episode revealed spike-wave paroxysms in parieto-occipital regions of the left hemisphere (Fig 1). During nonrapid eye movement (NREM) sleep, the EEG showed epileptiform activity of continuous spike-wave greater than 85% (Fig 2). The Panayiotopoulos syndrome can occur in very rare cases with atypical clinical and EEG findings, being the latter central in determining the patient’s prognosis1.

Sleep and epilepsy syndromes.
Sleep and epilepsy have a close relationship. About 20% of patients suffer seizures only during the night, approximately 40% only during the day and approximately 35% during the day and night. In certain epilepsy syndromes, the occurrence of seizures is strongly related to sleep or awakening. Infantile spasms appear predominately on awakening, and hypsarrhythmia is sometimes visible only in sle...
متن کاملEpilepsy with continuous spike-waves during slow wave sleep: a clinical and electroencephalographic study.
We report four children with epilepsy with "continuous spike-waves during slow wave sleep" (CSWSS). The main clinical features were partial motor seizures, mental retardation and motor deficit. The EEG findings were characterized by nearly continuous (> 85%) diffuse slow spike and wave activity in two patients, and localized to one hemisphere in two other cases during non-REM sleep. The treatme...
متن کاملAge-Dependency of Location of Epileptic Foci in "Continuous Spike-and-Waves during Sleep": A Parallel to the Posterior-Anterior Trajectory of Slow Wave Activity.
BACKGROUND Epileptic encephalopathy with continuous spike-and-waves during sleep (CSWS) occurs during childhood and is characterized by an activation of spike wave complexes during slow wave sleep. The location of epileptic foci is variable, as is etiology. A relationship between the epileptic focus and age has been shown in various focal epilepsies following a posterior-anterior trajectory, an...
متن کاملThe pathophysiological mechanisms of cognitive and behavioral disturbances in children with Landau–Kleffner syndrome or epilepsy with continuous spike-and-waves during slow-wave sleep
Epilepsy with continuous spike- and -waves during slow-wave sleep and Landau-Kleffner syndrome are two rare childhood epilepsy syndromes. The underlying pathophysiology remains unknown. The current opinions about epidemiologic risk factors, genetic predisposition, EEG and MEG findings, influence of anti-epileptic drugs, neuroradiology, positron emission tomopgraphy, evoked potentials, auto-immu...
متن کاملTreatment with flunitrazepam of continuous spikes and waves during slow wave sleep (CSWS) in children
We describe our treatment of two boys with continuous spikes and waves during slow wave sleep (CSWS). One of the boys was suffering from non-convulsive status epilepticus and the other from conscious disturbance with automatism. Their ictal EEG readings showed continuous diffuse spike and wave complexes, which were considered to show electrical status. The boys were diagnosed as having CSWS, an...
